Responsibilities

Work Schedule:
Work schedule shall be between 0700-1700 Monday-Friday to include a one-hour uncompensated meal break as determined by the clinical department.

 

Core Duties: The duties include, but are not limited to the following;

• Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established CM standards of care.
• Identifies and screens patients to determine needs and resources required for CM services. Performs comprehensive patient and family/caregiver needs assessments.
• Develops comprehensive care plans which address patient, family/caregiver’s desired or expected goals and outcomes, to include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
• Utilizes coordination, collaboration, and communication with the patient, multi and/or interdisciplinary team to implement the plan of care.
• Facilitates timely access to the appropriate level of care by coordinating referrals and transitioning the veteran to VA, DOD other federal, state and local home and community-based services.
• Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.
• Develop and implement tools to support CM, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
• Integrate CM and utilization management (UM) and integrating nursing CM with social work CM.
• Evaluates CM processes to determine interventions effectiveness, support continuous performance improvement and ensure a high quality and sustainable CMP.
• Maintain liaison with appropriate community agencies and organizations.
• Accurately collect and document patient care data in accordance with local policy and nursing standards of practice.
• Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
• Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.

Qualifications

Minimum Qualifications:

• Degree:Possess a Bachelor’s degree of Nursing.
• Education: 
Graduate from a college or university accredited by the Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE), or a national nursing accrediting agency recognized by the US Department of Education.

• Experience:Possess a minimum experience of at least 2 years of full time clincal nursing case manager after graduation. 

• Must have the knowledge and skills to effectively apply the following core CM functions:

o Assessment: Identification of patients for CM; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care.
o Planning: Collaboration with the patient, family/caregiver, primary provider, and other members of the health care team for developing an effective plan of care.
o Facilitation: Care coordination and communication among all involved parties.
o Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received.

• Must be knowledgeable in medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]); accreditation standards of Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC). 

* Certification:Possess one of the following certifications OR meet requirements under the experience section above.  Possess and maintain a current certification from one of the following accredited organizations: (CCM), (CDMS), (CRRN),(COHN),(COHN-S),  (ACCC), (CRC), (RN-NCM),(CMC) 

• Licensure:Current, full, active and unrestricted license as a Registered Nurse 

* Security: Must possess ability to pass a Government background check/security clearance.
* Life Support Certification: Possess a current AHA OR ARC BLS Healthcare Provider certification.
