Work Schedule:
Monday - Friday 8.5 hours between 7:00am and 5:30pm
No weekends and holidays
No On-Call or Call-Back
Core Duties:
Additional Duties assigned:
• Perform a full range of services within the scope of their competencies and the applicable duties provided in the basic contract.
• Maintain records of nursing care, dose and time of medication administered, and indicate if the medication was not administered and the reason.
• Perform preoperative procedures for minor surgery, and fill out pre-op checklist.
• Handle telephone information requests with courtesy, accuracy and respect for patient confidentiality. Receive information and distribute messages as necessary.
• Observe maintenance and supply of pharmaceuticals in the clinic, notifying the ordering officer when replenishment is required.
• The HCWs will address medical issues with the patient in a compassionate, comprehensive and integrative manner utilizing a collaborative approach with emphasis on coordinates care with the local clinic, utilization and referral management policies, and obtains additional clinical data, as required.
• Instructs patients and/or family members in self-care using clinic resources and ensure appropriate health care process instruction to patient and or caregivers based on identified learning needs.
• Identifies patients in need of care coordination through patient interaction, screening patients with a pattern of high risk, unstable and resource intensive utilization of MTF and network services.
• Prepare and submit administrative reports, to include complex coordination cases to the MTF and site management personnel routinely and upon request.
• Coordinates with network facility personnel and/or Managed Care Support Contractor personnel to ensure seamless transition of care back to the MTF for all patients hospitalized in the network or non-network facilities.
Minimum Qualifications:
• Degree: Certificate
• Education: Graduate from an accredited community college, junior college, college, university or vocational nursing program
•Certification: Possess a current AHA or ARC BLS Healthcare Provider certification
• Experience: One year of practical or vocational nurse experience after graduation
• Licensure: Current, full, active and unrestricted license as a practical or vocational nurse.
* Security: Must possess ability to pass a Government background check/security clearance.
